Guidelines for filling Online Registration Forms

  • This is an electronic registration form for Pravesh Vatika. Please read the instructions carefully before filling in the form to avoid rejection of your form.
  • The registration form will refresh while it retrieves dynamic data from our database.
  • Do not press the ENTER KEY to move from one field to another. Use the TAB KEY or the mouse control for the same.
  • On filling the father’s information, you will find a VERIFY button besides mobile no. field for mobile no. verification. After entering the mobile no. press TAB key. An OTP is sent on the mobile no. Fill the OTP number and click on the Verify button besides the OTP field. After the mobile no. verification, fill the remaining information. In case of single parent the same process is to be followed. However the details of only one of the parent/ guardian is to be filled.
  • After completing the application form kindly check all details and then click on the SUBMIT FORM button.
  • To avoid rejection, DO NOT fill more than one form for one applicant.
  • All fields marked with * are mandatory.
  • Please enter your email id and mobile number correctly.
  • When user selects values in some List box & radio buttons, the registration form will refresh for retrieving the data from the database. Till refreshing is completed, the user should not enter or select any data in the registration form as it may lead to loss of entered /selected data.
  • Kindly note that when you have a slow internet connection, the registration process will a take time.
